Fill this in real time, not from memory at the end of the shift. If it's not on this sheet and not in the CRM, per Module 12 it didn't happen.

A blank in the 'In CRM?' column at end of shift is not a paperwork gap — it's a lead the CRM doesn't know exists yet. Close every one before you clock out.
This sheet is scratch paper for your own discipline check — the CRM is still the system of record. If the two disagree, the CRM is wrong and this sheet just proved it.
